On the patient information page, you can download and complete the intake forms and bring them to your initial visit. At your first visit, Dr. Ambrocio will carefully review your history, perform a thorough physical examination, and recommend a diagnostic and treatment plan. You will have the opportunity to ask questions and discuss your concerns. Results of x-rays, laboratory tests, and other imaging studies are discussed at the subsequent visit.
